Once registration is complete, the student’s rights may not be assigned to any other person.
In consideration of being accepted as a student of CHINESE MARTIAL ARTS, my heirs or executors herewith release and absolve CHINESE MARTIAL ARTS, its agents, and / or employees from any and all responsibility or liability in the event that I receive any injuries sustained during instruction or practice, or of any injuries by me or about the premises due to accidental cause.
